When is it cliche to have an insane villain in a story?

When it becomes unrealistic. Don't make him or her like a cartoon character. Decide certain traits that he has because of his insanity or better yet, research and give him an actual disorder/disease. If you do that, you will have traits and things to go on that will keep your villain's insanity on track ;)
